What you are looking to do is called "Kiosk Mode". If you decide to go with a laptop, here are some instructions about locking down Chrome to show only your form. If you choose to go with an iPad, it looks like these instructions might help you lock down Safari (be sure to read the link about Guided Access, which lets you set the iPad to display just one app).
